Moderna’s vaccine against the Omicron variant BA.4/5

*The omicron vaccine is valid for people who received the first and second doses, and can be administered once per person regardless of the number of vaccinations.

Target Audience (Vaccination for third, fourth and fifth dose)

Persons over 18 years of age who received the previous dose more than 3 months ago and reside, work and/or study in Mie.

*Limited to those who have been vaccinated with any of the vaccines from Pfizer, Moderna, AstraZeneca and/or Takeda (Novacs).

What to bring on the day if you have the vaccination ticket (sesshuken – 接種券)

1. Vaccination ticket sent by the prefecture (if your ticket is a stamp, click here to download the Yoshinhyo – 予診票)
2. Preliminary Exam Sheet – Yoshinhyo (fill in the required items)
3. Documents, applications and other items that prove the vaccination history
4. Identification document (driver’s license, kenko hoken-sho, etc.)
5. Medicine booklet (O-kusuri Techo – お薬手帳), if you have it
